Instructions for obtaining a Background Check

1- Click the button to go to the Authorization Form.

2- Download, print, complete and properly sign the Authorization Form.

3- Click the button to go to the List of Available Services.

4- Download, print and check the box or boxes to the left of the services that you want.

5- Return pages 1, 2, and 3 of the Authorization Form and pages 1 and 2 of the List of Available Services form.

An itemized invoice will be generated and returned to your designated e-mail address.
The Background Check cannot be completed without the Authorization Form filled out, signed and payment being made.   Payment instructions will be returned with the itemized invoice. CRS is proud to use PayPal.
